Disease Manager
Role and Duties:
- Promote the mission and core values of Quantum Health
- Assess all the requests for medical services requiring clinical review against Quantum Health adopted clinical review criteria
- Provide case management for enrollees identified as having one of the chronic conditions being managed by Quantum.
- Assess each case presented and utilize the condition specific Care Path to educate
- Assist patients in planning health goals and provide resources to help them execute goals
- Document all case management activities specific to interventions for your patients
- Be a clinical resource for Clinical Services, Patient Services, Case Management, Wellness, Account Coordinators, Account Executives, and Stop Loss Coordinator.
- Coordinate all services in an effort to provide integrated health services for each patient and provide benefit and health information to each patient so they are able to make informed health decisions.
- Monitor compliance for each patient
- Evaluate the response to disease management intervention and report outcomes to Director of Clinical Services
- Provide group specific data to the Manager of Disease Management for purposes of reporting.
- Provide aggregate data for groups specific to types of condition, dollar amounts, and level of acuity
- Provide Case Management to groups not assigned Case Managers
- Coordinate health management services in an effort to provide integrated health services for each patient and provide benefit and health information to patients, care givers, providers and facilities so they are able to make informed health decisions
Tasks:
- Identify cases appropriate for Disease Management
- Organize contact of individuals identified based on acuity
- Document all contacts in database
- Document Interventions related to patient, care giver, provider, facility, or client in the database
- Have working knowledge of all reports and guidelines related to Disease Management
- Provide monitoring and oversight to non-clinical staff activities and be available to non-clinical staff during CHC business hours
- Perform Case Management as needed
- Have knowledge of all reports and guidelines related to Case Management, Utilization Management and pre-certification

Helpdesk Administrator
This role will focus on the accurate routing of helpdesk tickets to other groups. This role will answer the helpdesk phone and attempt to resolve user’s issues as a level 1 support option. Role: Maintaining the integrity of the database especially related to eligibility. Serving as a resource for generating reports from the database on eligibility. Providing general support on database and reporting software as requested for other working groups.

Case Manager
The Case Manager is responsible for providing telephonic case management services for enrollees of Coordinated Health/Care employer groups who have catastrophic illness and/or injuries. They also function as a clinical resource for other staff in the company such as utilization review, patient services, disease management and sales.
- Responsibilities/Tasks:
- The duties of the RN Case Manager may include, but are not limited to the following:
- Identification of employees who may be appropriate for case management
- Research and review of the identified employee followed by telephone contact, if appropriate
- Provide case management services for appropriate employees who are willing to participate in the case management program
- Perform tracking and reporting of case management activity
- Perform post discharge calls
- Maintain contact with Utilization Management staff to assist in discharge planning for CM cases
- Maintain contact with the Disease Case Management staff for transfer of cases when appropriate
- Provide clinical updates to the Stop Loss Coordinator as requested
- Act as clinical resource for all departments
- Maintain a working knowledge of all policies and procedures related to Case Management and Coordinated Care Services
- Maintain a working knowledge of all benefit plans and know where to access the needed information
- Negotiate for services needed if no clear benefit exists or if the services can only be provided out of network
- For reviewing and understanding the scope of practice in the states where licensed.

- Skills/Education:
- Licensed as a Registered Nurse in Ohio
- Must have one or more of the following:
- 3 years of varied and extensive experience in medical-surgical nursing
- A bachelors or higher degree
- Certification in Case Management

Utilization Management Nurse
- Roles:

- Assess each request against adopted clinical review criteria
- Conduct pre-review screening, initial clinical review, concurrent review, and retrospective review
- Facilitate in discharge planning process pre-service, during concurrent review, and post discharge, this includes but is not limited to: making arrangements for skilled nursing care, rehabilitation, home infusion and other out patient ancillary services
- Utilize Quantum Health adopted clinical criteria during all aspects of the review process and document outcomes in authorization database
- Re-direction of out of network requests to in network requests whenever possible to ensure members obtain the optimum benefit
- Negotiate fee arrangements with out of network providers on a case by case basis
- Facilitate communication between medical provider including external review agencies
- Notify Medical Director of all potential non-certifications within the appropriate time frames
- Provide monitoring and over-site of non-clinical staff activities and be available to non-clinical staff during CHC business hours
- Responsible for the reporting process which encompasses varied workflows depending upon employer groups
- Responsible for review of diagnoses making appropriate referrals to CM/DM when appropriate
- Maintain a working knowledge of all CHC workflows

- Prepare letters explaining non-covered requests
- Document Interventions in the database
- Direct out of network requests to in network providers to ensure members secure the optimum desired benefits and negotiate fee arrangements with out of network providers as needed
- Facilitate the discharge planning process during pre-service review and coordinate with other clinical work teams
- Notify Medical Director of all requests you are unable to certify within specified time frames
- Provide monitoring and oversight of non-clinical staff activities and be available to non-clinical staff during CHC business hours

Patient Service Representative (PSR)
Role: Patients Services Representatives (PSRs) are the first and most frequent contact with patients, families, providers, and office staff who utilize Quantum Health’s coordinated healthcare program. PSRs take approximately 45 to 70 phone calls per day. Their role is to serve as patient and provider advocate by answering all specific questions, while educating and guiding patients through the complicated process of healthcare. PSRs also follow-up on calls by completing research, solving problems, and contacting various parties to implement solutions. Compared to a typical health plan call center, this position is more intensely focused on problem solving, patient advocacy, education and guidance.
Roles & Tasks: The POD Manager is responsible for determining the specific tasks to be performed by this position. These tasks include but are not limited to:
- Taking phone calls and/or internet inquires from patients, families, providers, and employer/client representatives and responding to inquiries regarding benefits, eligibility, claims, provider look ups and care coordination processes.
- Completely documenting all contacts with any of these parties.
- Coordinating activities between patients, providers, TPAs, PBMS, and any party impacting the healthcare of a patient, by acting as an advocate for the patient and assisting them to understand their healthcare processes and applicable benefits.
- Assist providers and members with the referral and pre-certification process through coordination of care. The PSR will not be responsible for conducting any utilization or pre-certification activities which require interpretation of clinical information.
- Identify and escalate issues as appropriate.
Skills: This position requires the following skills/education:
- Minimum 2-3 years experience as a social worker in a healthcare setting familiar with self-insured benefit plans, or 2-3 years experience as a customer service representative or claims adjudicator; or 2-3 years experience working in a healthcare setting familiar with self or fully funded benefit plans
- Empathetic attitude for patient and providers when addressing issues or concerns;
- Ability to take 45-70 incoming calls per day and deal with high-pressure, often highly emotional situations while maintaining a positive attitude;
- Ability to probe to determine root cause of problems to facilitate resolution of issue;
- Strong customer service skills and/or superior phone skills;
- Ability to make decisions that are timely and well thought out that achieve results consistent with business goals and organizational culture;
- Ability to function as a team member with the working group and to interact professionally with other entities both internally and externally;
- Ability to work independently and self-directed with limited need for day to day supervision.
